Patient Friendly Medicine Information Leaflets

The Trust now has access to a database (MaPPs) of patient friendly information about medicines that will make it easier for patients to understand their medication. The handy summaries make information much clearer and simpler for patients, ensuring they leave our care confident about what their medicines are for and how to take them.

All patients with changes to their medicines will be given an information leaflet before being discharged. The patient (or carer) will also have the changes explained verbally. The leaflet will be given to the patient to take home so that they can read the information again at their leisure. The handy summaries don’t replace the package inserts and they refer the patient to the longer leaflets.

This is the latest in a succession of improvements on discharge information about medicines. The electronic discharge summaries make the information much more legible and we can send them to GPs by email or electronic message.
