SASH@home

Our hospital at home service

SASH@home benefits patients by supporting them to leave hospital as soon as they are clinically stable, enabling them to complete the remainder of their acute nursing and/or therapeutic care in the comfort of their own homes. Home treatment is safe and effective and, most importantly, gives patients more choice and control over their care.

The SASH@home clinical team

The team is made up of nurses, physiotherapists, occupational therapists, rehabilitation assistants and healthcare support workers. The clinical team travels to a patient’s home to provide care.

Patients accessing SASH@home remain under the care of their SASH consultant team.  Review processes are established with the SASH consultant teams before patients are transferred to the home service.

The patient’s home environment, equipment needs, domestic support and medication requirements will be reviewed during an initial assessment carried out by the SASH@home clinicians before patients are transferred to the service. If the home environment is not appropriate or it is felt that care cannot be delivered safely in the patient’s home then hospital-based care will continue.

Hours of operation

SASH@home clinicians work seven days a week between 7 am and 10 pm.

If patients or their carers/relatives need support outside of the arranged visit times, there is a 24-hour on call service available. Call 0800 028 9677 for out of ours support.

For clinical enquiries or to refer to SASH@home:

  • Monday to Friday call x 2041 or bleep 2042
  • Out of hours call the Healthcare at Home Care Bureau on 0333 103 9813

 

The SASH@home service is accessible for all specialties to use if consultant teams feel they have an appropriate patient (except paediatrics).
