Respiratory Outreach

There are two Respiratory Outreach Teams with the Trust which provide ventilation support and palliative care to patients. The East Surrey Hospital-based team provides support and palliative care to all patients within the SASH geographical area with respiratory disease and those with Motor Neuron Disease needing ventilation support, plus all patients within the East Surrey community with Chronic Obstructive Pulmonary Disease (COPD) as well as all in-patients at East Surrey Hospital.

The Crawley Hospital-based Respiratory Outreach team provides support and palliative care for COPD patients only, within the Crawley area.

Both teams run a Pulmonary Rehabilitation Course. This is an 8-week course with a personalised exercise programme and lung advice for patients with COPD or other chronic lung conditions who have reduced exercise capacity. The courses take place at the East Surrey Hospital gym, Oxted Health Centre and in the gym at Crawley Hospital.


All GP Referral Letters, except those referred though the Choose & Book System or Two Week Rule method, should be sent direct to the Outpatient Booking Office, East Surrey Hospital, Canada Avenue, Redhill RH1 5RH. Please do not send these direct to a consultant, medical secretary or any other department. You can of course make the actual outpatient referral letter out to a specific consultant. The Outpatient Booking Office is responsible for processing ALL referrals for outpatient appointments which then initiates the patient’s 18 week RTT status.

Outpatient Referrals made under the TWO WEEK RULE method should be faxed immediately to 01737 231733.

Pulmonary Hypertension CNS community support

Service covers SASH locality and offers specialised support in the comfort of your own home. Mon – Fri 08.30 – 16.30

Patients will be highlighted through clinic referrals

• Coordination of investigations re: Pulmonary Hypertension
• Support surrounding initial diagnosis and on-going support once a PH diagnosis is established
• Support and advice given re: Lifestyle changes
• Exercise – the service also refers into and works with the Pulmonary Rehabilitation Services
• Oxygen assessment and long term management
• Advice on medications
• Information on Benefits and support in applying where appropriate
• Breathlessness management
• Anxiety management/ emotional wellbeing

Home Ventilation Service

Patients who require home ventilation support will be identified either when they are an inpatient at East Surrey Hospital or through one of the outpatient clinics. The department provides all the equipment for non-invasive (nasal or full face) home ventilation, and our Outreach Respiratory Team provides assistance to patients within the community.
